REFERRAL FORM

In order to be admitted into the program, the client must be referred by a mental health professional such as a licensed therapist, has to be receiving ongoing clinical services at the time of referral ,or must have a diagnosis indicating a severity of psychiatric symptoms that  needs psychiatric rehabilitation services.
The agency works collaboratively with other community based mental health programs, Department of Juvenile Services, Department of Social Services, Maryland Court Systems, and social worker to identify those clients in need of immediate psychiatric rehabilitation interventions.
